Liability Waiver 

Top6ix 1V1 Tournament: Consent and Liability Form

I, hereby acknowledge and agree to participate in the Top6ix 1V1 Tournament, organized by Top6ix Staff.

1. Acknowledgement of Risks: I understand that participation in soccer involves inherent risks of injury, including but not limited to, strains, sprains, fractures, and concussions. I voluntarily assume all such risks.

2. Medical Fitness: I certify that I am physically fit to participate in the tournament. I agree to disclose any medical conditions or injuries that may affect my ability to safely participate.

3. Consent for Treatment: In the event of an injury or medical emergency, I authorize tournament officials to obtain medical treatment deemed necessary for my well-being.

4. Liability Waiver: I release the organizers, sponsors, and their respective officers, employees, and agents from any liability for injuries or damages arising out of my participation in the Top6ix Soccer Tournament.

5. Photography and Publicity: I consent to the use of my name, likeness, and images captured during the tournament for promotional and publicity purposes by the organizers.

6. Code of Conduct: I agree to abide by the rules and regulations of the tournament and to conduct myself in a sportsmanlike manner at all times.

7. Parental/Guardian Consent (if participant is under 18): I, as the parent/guardian of the participant, hereby consent to their participation and agree to all terms outlined in this form on their behalf.

 

I have read and understood that by registering for any Top6iX event, I consent to the terms of this liability form, and I willingly agree to be bound by them.
